7.29.3.1 Appendix does not appear in PDF output

\end{document} 
When I compile this and create a PDF with command pdflatex foo.tex &&... 
[appendix]

7.29.3.2 Full page figure in landscape alignment

I want to have more control over the placement of a figure in a landscape orientation. For some reason the usual options (t, b, etc.) have no effect in my document. Latex just places it at the bottom ... 
[landscape] [pdflscape] [lscape]

7.29.3.3 Using titlesec how can I prevent space from showing up between chapters and sections if I do not have text between them?

I am stuck with a very strict requirement to keep the space between chapter/sections/subsections/paragraph headers the same.  After a chapter header, I need to have a triple space.  After section ...
